Premier Plan (Monthly Telemedicine & Monthly House Calls Membership) $279/month
- Telemedicine Services. Telemedicine Services provided in this Agreement are those Services that are consistent with Physician’s training and experience, and as deemed appropriate at the sole discretion of the Physician. The Patient is responsible for all costs associated with any medications including vaccinations, laboratory testing, durable medical equipment, and specimen analysis associated with these Services. The Medical Services provided under this Agreement include the following:
- Unlimited 15-minute increment Telemedicine Virtual Visits per month.
- House Call Services. Medical Services provided in this Agreement are those Services that are consistent with Physician’s training and experience, and as deemed appropriate at the sole discretion of the Physician. The Patient is responsible for all costs associated with any medications, laboratory testing, durable medical equipment, and specimen analysis associated with these Services. The Medical Services provided under this Agreement include the following:
- One (1) House Call Visit (Acute & Non-Acute Visits) every month and not to exceed Twelve (12) House Call Visits in any consecutive 12 months span.
- Each additional House Call Visit occuring after your One (1) Monthly allotment will be charged $150 per visit.
- The Medical Services provided under this Agreement include the following:
- Chronic Condition Management (e.g., diabetes, high blood pressure, high cholesterol, etc.)
- Comprehensive Annual Exams
- Preventive and Wellness Exams
- Sports Medicine
- Medical Weight Loss and Weight Management
- Body Fat analysis
- Hospital follow up care
- Preoperative evaluations
- Sports physicals
- EKG
- Smoking cessation
- Spirometry
- Nebulizer treatments
- Removal of benign skin lesions/warts (excluding pathology fee)
- Ingrown toenail removal
- Skin biopsies (excluding pathology fee)
- Simple wound repair and sutures
- Simple aspiration/injection of joint (excluding any pathology fee)
- Abscess Incision and Drainage (excluding any pathology fee)
- Removal of Cerumen (ear wax)
- Administration of Injectables (additional fee for cost of products / medications)
- “Annual Labs” Free of Charge. This includes CBC, CMP, HbA1c, Lipid Panel, TSH & FT4, Vitamin D Levels, hs-CRP & Homocysteine, & a Compete Urinalysis. PSA is included for age-appropriate males. Other lab testing at significantly reduced rates through select vendors as negotiated through the Practice.
- Access to Mobile Phlebotomy at significantly reduced rates through select vendors as negotiated through the Practice.
- Annual Chest X-ray Free of Charge. Access to imaging at significantly reduced rates through select vendors as negotiated through the Practice.
- Access to a variety of prescription medications at wholesale or close to wholesale price and dispensed directly by physician.
- IV Nutritional therapy (additional fee for cost of products / medications) but includes a Twenty Percent (20%) discount on all products / medications.
- Medical Aesthetics (additional fee for cost of products / medications but includes a Twenty-Five Percent (25%) discount on all products / medications.
- Hormone replacement therapy (additional fee for cost of hormones)
- Physical Therapy (additional fee for these services)
- Four (4) Massage Therapy sessions Free of Charge per year (One (1) every Three (3) months. These are sessions of at 60-minute increments for a total of Two Hundred Forty (240) Minutes total. Additional fees for additional sessions.
- Annual Age-Appropriate “Flu” vaccinations free of charge
