Essential Plan (Monthly Telemedicine Membership) $69/month
- Telemedicine Services. Telemedicine Services provided in this Agreement are those Services that are consistent with Physician’s training and experience, and as deemed appropriate at the sole discretion of the Physician. The Patient is responsible for all costs associated with any medications including vaccinations, laboratory testing, durable medical equipment, and specimen analysis associated with these Services. The Medical Services provided under this Agreement include the following:
- 2 Telemedicine Virtual Visits in 15 minutes increments per month totaling a total of 30 minutes in duration.
- Each subsequent 15-minute increments will be charged at $50.00/15 minutes.
- No Free House Call Visits per month or year.
- House Call Visits charged at $231 per visit.
